Social and Community Participation

We believe in fostering connections and enhancing the social lives of older and vulnerable individuals by supporting their engagement within the community. Our services are designed to assist with accessing social events, developing new hobbies, and participating in group activities, all aimed at promoting independence and enriching social interactions.

Social and Community Participation

Community Engagement: Connecting Hearts, Empowering Lives

Research continuously shows us that loneliness and isolation are main contributors towards suffering in older and vulnerable persons. That is why we have dedicated a number of services specifically to help you continue to participate and contribute towards the community you have been involved in over the years.

We also know that for some other persons, achieving or regaining the independence to live and integrate in the community can be a challenge sometimes.

We therefore offer services such as:

Add-ons

What are Add-ons?

Add-ons are services which are available at an additional cost as these are delivered by third-party professionals. Prices vary according to service needed and client will be presented with a quote prior to service delivery.

Add-on Social/Community participation services include:

free assessment

How can I access these services?

Call us and set an appointment for an Initial Assessment. We will help you decide which package is best for you. Or if you know exactly what you need already, you can create your very own package, with exactly the right combination of personal care, assistance and interventions you require.

With you, we will set your wellbeing goals and create your care plan and you can start receiving our services immediately!

Add-on services are available to existing clients who are already availing themselves of our services and therefore do not require an additional assessment. They need to be booked in advance, just as you would a hairdresser, beautician or cosmetologist.

Is my family involved in the process?

Your family, or any person of your choice can be kept updated with what is happening during the sessions, as long as this is authorised by you or a legal guardian. Our software system allows family members to log onto any part of your care records (according to clearance given by you) so they can follow activities, services and even your progress on the wellbeing goals we set together!
